_______ Your new puppy has received the above vaccinations.  It is very important that you continue to
vaccinate your puppy every 4 weeks for the next 2-3 months or as recommended by your Vet.

_______  Seller guarantees health on puppy for the first seven days.  It is mandatory that the puppy be taken to
your local Vet within 48 hours of purchase.  Seller is not responsible for any Vet fee that may incur. This is the
purchaser's responsibility.  If the puppy does not get its mandatory Vet check-up, this guarantee is NULL and
VOID.  This guarantee does not cover intestinal parasites or common treatable ailments such as coccidiosis.  
Should your Vet find that the above puppy has a genetic defect within one year, the puppy will be replaced
with the next available puppy at equal value only with a statement by your Vet.  Such defects covered under
this guarantee are limited to those which are determined by a licensed veterinarian to be (1) terminal in nature;
(2) will require non-elective surgery(ies) and/or medication(s) in order to correct or prolong life;  (3)
Significantly and substantially diminish the quality of life. A second opinion may be required to verify this
opinion.
